Providers’ credentials have been used to create fraudulent prescriptions for Oxycodone and Promethazine with Codeine in Eastern Iowa.

That’s according to a news release from the Iowa Board of Pharmacy, which says convincing security paper is being used and the violators are using the tag “software failure” on the prescriptions to avoid electronic prescribing. On some occasions, a provider’s license number from a state other than Iowa is being utilized.

Currently, pharmacies impacted are in Eastern Iowa. Law enforcement and the DEA have been notified.
